We're reviewing the case
of M.C.,
a 30-year-old male who presented
to ED last week.
Dr. Apostolou
is joining us from Cardiology.

you were treating him.
Can you walk us through
exactly what happened?
I absolutely can.
Mick came in around midday
with an 8cm laceration
on his left thigh
from roofing material.

And what was your assessment
of his overall condition?
Very healthy.
[Jimmy] No history,
no allergies, no meds.
Did you do
a cardiac examination?
[Jimmy] I listened to his heart.
[Apostolou] Can you describe
exactly what you heard?
[Jimmy] Regular heart rate
and rhythm.
No murmurs.
Another one of my patients
had run off,
so I was just making sure
that everyone

Dr. Seufale had approval
to discharge the patient.
Instead, he was found
in cardiac arrest
in a hospital bathroom.

Resuscitation was attempted.
He was pronounced dead
at 4:23pm.
[Apostolou] Autopsy revealed
an undiagnosed genetic heart
condition,
hypertrophic cardiomyopathy,
with severe left ventricular
outflow tract obstruction.
There was also evidence
of a recent stress response,
elevated catecholamine levels.
[Gloria] Did you consider
how trauma
can unmask an underlying
heart condition?
A cardiac exam
was not indicated here.

Our triage protocols
rely on the doctor's judgement
for the extent of physical exam.

in a blood vessel,
and over time,
the pressure builds up
and it can stretch
and it can burst.

One of them is near
the part of your brain
which controls your breathing
and heartbeat.
The other is
also extremely dangerous.
Okay, so what do we do?
[Curtis] Well, we're gonna
refer you to Neurosurgery
and they'll take you through
your options.
Surgery is the only option.
The risk of a rupture
is extremely high.
[Melissa] Okay, well,
what does surgery involve?
Well, what they'll do is they'll
try and clip the aneurysm
or they'll block it up
from the inside
with tiny little coils.
But because of where they are,
right near the brain stem,
there's quite a considerable
risk with the procedure.
What does that mean?

- It means you could die.
