- [EMT] 32-year-old male.
Tried to beat a speeding train,
didn’t make it.
Severe neck and back pain.
Deformity to the right lower leg.
His vitals are stable.
[Danny] It's Glenn Vogel.
- The guy I got in a fight with.
- [Phillips] You're kidding.
Wife and baby were in the car with him.
They're on the next chopper.
All right. Let's get him downstairs.
[intense music playing]
- [Elijah] What have we got?
- [Phillips] Patient's name is Glenn Vogel.
- Thirties, in a car hit by a train.
- Glenn Vogel?
Are you kidding me?
I do not want her in here.
Sir, I need you to lay back down.
It is not your decision.
You neither! Get out!
- Leave me alone and let me--
- No.
[Glenn screaming]
- You need to relax. You need to relax.
- [Danny] You have very serious injuries.
If your neck or back is broken
you could end up paralyzed. Okay?
I don’t give a shit! Out of my face!
Hey! Stop! Or we'll put you in restraints.
Don't look at me. Let us treat you,
or we park you in the hall,
and you can call someone to pick you up.
[tense music recedes]
Fine.
Fix me.
- What do you wanna do first?
- Um, Danny, heart and lungs.
- Then let's get a FAST going.
- On it.
Harper, check out his legs for me?
Obvious deformity
of right lower extremity.
Other than your leg,
can you tell me what else is hurting?
Where’s my wife? My baby? Where are they?
We heard they're coming in
on the next chopper.
Are they hurt? Are they gonna die?
If they’re bringing them to this hospital,
it's because they’re alive.
His wife and baby got here.
They’re coming down.
Okay. We got him. Go.
No. Wait. What happened to them?
Are they okay?
I’m going to find out.
Stop! I don't want you touching them
without talking to me first--
I know that you don’t like us, but he is
who you want taking care of your family.
[Glenn] That’s my wife.
[tearfully] My baby.
[crying] Please do not let them die.
Please don't let them die!

Hey, Marco, were you able to get
those LFTs and coags on bed nine?
Oh, man, I'm sorry.
I am so backed up still.
Speaking of which,
watch out for Dr. Broussard.
She's reaming out anybody
who takes too long on a discharge.
- God forbid her metrics suffer.
- Yeah.
Doctors Phillips and Morales,
we've got 27 in the waiting room.
I hope you're not talking sports.
No. I've actually just discharged
one and three.
Then you can take
rooms four, six, and seven.
Dr. Broussard, I've got
a full load of patients already.
Then work faster, golden boy.
Julio, how you doing, bud?
- [Julio] Hey, Dr. Phillips.
- What's going on, man?
- Sandra, good to see you.
- Good to see you.
- How you feeling?
- I'm okay.
It's a lot of fluid,
and it's pretty painful today.
- Let's take a look here.
- [Julio grunts]
A lot of ascites.
Any change in your liver disease?
Not really.
Have you been seeing Dr. Forman?
Going to your GI appointments?
Um… I’ve, uh, been going to FIU now,
and, uh, my schedule’s really busy.
Hold on, FIU? When did that happen?
I feel like you just started high school.
Tell me about it. He's the first person
in the family to go to college.
- That's awesome.
- Thanks, man.
So there’s not a lot of tenderness.
Have you been having any fevers?
No, but the pressure…
I mean, it's hard to breathe.
Can you take some of the fluid off?
You’re really supposed to
get it done in your GI clinic,
and we’re really slammed today.
But, uh… yeah, let me see what I can do.
Sit tight. I'll be back.

Dry mucus membranes, respiratory distress.
- I can take Benny. I saw him days ago.
- Keep me posted. Cass, stay with her.
- Let's go to exam room three.
- Yeah, sure.
- Let's get her to trauma one.
- Yes, Doctor.
- Excuse me, ma'am, what's your name?
- [labored breathing] Kathryn.
Kathryn we're gonna take good care
of you, all right?
What have we got?
Nine-month-old involved in a car accident.
Tachypneic and dry mucus membranes.

so you're gonna run point with me, okay?
Let's figure out a battle plan.

what happened with Soriano.
- Yup.
- How bad is it?
For you or for me?

For being with me?
For believing in you.
Look, I'm sorry, I…
That's fine.
And for me, at least,
it's gonna pass, but you…

but you're making it quite difficult.
Not difficult enough, apparently.
This is awkward for the both of us. Okay?
You don't want to give up the department.
- I'm not trying to take it away from you.
- Oh, really?
You're not actively trying
to campaign for the job?

I wanna work with you.
In fact, I was coming to ask you
who you've chosen
for Chief Resident next year.
Well, when the board calls
to make it official, I will let you know.
There's one more thing.
Glenn Vogel and his family are in the ER.
Car crash. Danny Simms is treating him.
Just thought you should know,
considering what happened
a couple days ago.
[pensive music plays lightly]
Uh, bedside echo looks good.
- You have 60%.
- Okay.
- What's the update?

We're working on the baby,
mother is pale and hypotensive.
Okay. Thanks.
[Glenn] What's going on? Are they okay?
They're… alive,
and we are doing everything that we can.
That's what the doctors
in Gainesville said to my dad
right before he died of a MRSA infection.
And we got a $170,000 hospital bill.
Wiped out everything they had.
I'm sorry.
Couldn't do anything for Benny
a couple days ago.
Why should I believe you now?
Are you saying Benny got worse
when he got home?
- Much worse.
- Dr. Simms literally begged you to stay.
We wouldn't be here now if you'd listened.
It's okay. We understand.
[somber music playing]
Maybe what you people did made him worse.
He stopped breathing so many times.
Kathryn was so scared.
I was trying to get him to my mom.
She's a midwife. She knows what to do.
So, that's why you were speeding.
It wasn't my fault.
Kathryn was screaming.
Benny had stopped breathing again.
Why couldn't you have helped him?

- Yeah, just a water, please.
So, uh, what happens now?
She wakes up,
her family could sue, and I'd be fired.
Or she dies,
her family could sue, and I'd be fired.
I'd also be devastated.
You really care about her.
I can't even admit that
without getting in trouble.
- Got you a new one.
- Oh, thank you.
You read my mind.
You are welcome… Chief.
Oh, you don't know that. That's…
I mean, it's going to be you.
And you deserve it. Honestly.
Just want you to know that.
Thank you.
So, Sicily, huh?
- Yeah.
- Mm-hmm.
I mean, why not?
[Danny laughs]
- Where would you go?
- I don't know.
Sicily sounds pretty good.
[Cruz] Hello, everyone.
Buenas tardes.
So good to see you all here.
It's been a great year.
And you have all worked tirelessly.
And for that, I cannot thank you enough.
I need to announce that next year,
there will be a change.
The administration has made the decision
to split the ER and surgical departments.
Which means I will remain
chairman of surgery,
and the ER will be getting
a new interim chairperson.
It really has been the honor of my life
to grow this hospital into
the trauma center it is today.
But there comes a time to pass the baton.
And I'm very pleased
to pass on the leadership of ER
to one of our own,
Senior ER Attending Patrick Sanchez.
[scattered chattering]
[light music playing]
[Sanchez] Thank you, Dr. Cruz.
I am truly honored
and humbled to take over this ER,
one which you have built
into such an incredible institution.
As most of you know,
I like to keep things simple,
and I don't like public speaking,
but there is one outstanding matter
on which I need to speak to you today.
And that is the appointment…
of our new Chief Resident
for the following year.
Now, in consultation with Dr. Cruz
and the board, I have decided to choose…
Dr. Samuel Elijah.
[group clapping and chattering]

Danny, I'm so sorry.
I didn't know he was gonna do that.
You don't have anything to be sorry for.
[Cruz sighs]
This is the price
you paid for me, isn't it?
Sanchez taking the ER?
It's not on you.
I was taught you take the hits.
You don't complain.
My mistake was thinking
that would stop the hits from coming.
They never stop coming.
You need to work on your pep talk.
[both laughing]
- [Cruz sighs] Ah…
- [Danny chuckles]
I'll be fine.
And you'll be fine too.
You impressed a lot of people
as Chief, Danny, not just me.
So don't underestimate yourself.
Because in the last couple of weeks,
you put yourself on the map.
Is that a better pep talk?
[both chuckle lightly]
I just cannot believe it.
- Of all the people they could have chosen.
- Hang on. There he is.
[Phillips] Hey, Patrick.
Congratulations.
Thank you so much, Dr. Phillips.
And also, um, there are
a lot of attendings I like less than you.
- [Cole snickers]
- [Sanchez chuckles]

- I really--
- Hey, I just…
- You go.
- No, you… You.
It meant a lot, what you said yesterday.
And I'm gonna miss our Sunday dinners too.
And I want you to know that I'm not gonna…
I'm not gonna tell anybody about Kennedy.
Well, it doesn't really matter.
I went to the hospital review board
this afternoon.
Told them everything.
- What?
- Yeah.
You could get sued.
You could lose your license.
- What about your parents?
- I don't give a shit.
It was the right thing to do.
So I did it.
This, uh, girl I really like
said that one time.
I'm trying to be a bit more like her.
