The Cobalt Fissure

1
Previously on Watson
Remember Alexander Petrov,
the Cobalt Fissure?
It was a three-day pursuit
that ended with us surrounded
by soldiers in biohazard gear on some
weapons testing site.
I think you're sick, Sherlock.
Beck, why are you pretending
to be Sasha's mother?
BECK: She cares about something,
maybe she smiles a lot less.
I was hoping to have
a quick word with Dr. Derian.
I gave my statement already.
Just want to review a few details.
MARY: John, you have a glioblastoma.
You're a hallucination.
You come from a traumatic
brain injury I sustained
when I went over that waterfall.
- You're not there, you're dead.
- You sure it's true?
So the real Sherlock
is just out there somewhere?
Guv? Are you all right?
Do I know you?
John, you're sick.
We are going to drive
to Baltimore to ask Dr. Niles
to get on his surgical calendar.
MAN: "'Twas brillig,
and the slithy toves
"Did gyre and gimble in the wabe:
"All mimsy were the borogoves,
"And the mome raths outgrabe.
"Beware the Jabberwock, my son.
"The jaws that bite,
the claws that catch."
NILES:
You do realize I'm admitting you
to the hospital today?
- (PHONE BUZZING)
- No, this was just a consult.
John, your phone. Maybe?
- You need surgery.
- (PHONE CONTINUES BUZZING)
This tumor's aggressive.
It's growing fast,
and it wants you dead.
Before it does that,
it's going to give you pain,
memory loss, seizures.
Listen to me.
Listen to me.
I know you.
I know who you are.
I know what you've done.
I know what makes you you.
You have a glioblastoma.
It's bad.
If you want to come out of this
still yourself,
still John Watson,
then you are in
exactly the right place.
But you need to trust me.
And we need to admit you.
Tonight.
HOLMES: There were two men.
Holmes and Watson. Watson and Holmes.
And they did
the most extraordinary things.
CARLIN: Are you one of those men?
Are you Sherlock Holmes?
I don't know.
There was a blue carbuncle,
hidden inside a goose.
Let's assume we're not all
having a mass hallucination.
Mm. That is Sherlock Holmes.
Doesn't know where he is.
He doesn't know how he got here.
The man don't even know his own name,
- but that is Sherlock Holmes.
- SASHA: He's in bad shape.
Dehydrated, hypotensive, fever of 102.
The triage nurse said he was
stumbling when he came in,
like he was drunk.
Wernicke's encephalopathy?
Sherlock Holmes is not an alcoholic.
You don't know what he is. Not now.
He might be malnourished.
Let's get him on thiamine.
Good call. Keep me posted.
Got to get back to Lauren, but, um
what about Watson?
Do we tell him?
I texted the man, but no response.
SASHA: Ingrid.
How'd it go with Detective Lestrade?
INGRID: It was fine.
We talked more about my father
than we talked about Beck.
Your father?
She had questions about his
"sudden disappearance."
She really liked those words,
"sudden disappearance."
According to Detective Lestrade,
I am a bad person
to be a bad person around.
She doesn't have anything
except for questions
that will never be answered.
Right, but your father,
that was one thing.
Beck was self-defense.
We have a patient.
(PHONE CHIMING, BUZZING)
(INDISTINCT P.A. ANNOUNCEMENT)
MAN: "And, as in
uffish thought he stood,
"The Jabberwock, with eyes of flame,
"Came whiffling through
the tulgey wood,
"And burbled as it came.
"One, two. One, two.
"And through and through
"The vorpal blade went snicker-snack.
"One, two.
One, two."
(INHALES SLOWLY)
(EXHALES SLOWLY)
"One, two."
- (GUNSHOT)
- (PEOPLE SCREAM)

SASHA:
Luna Quinn. Did anyone know her?
She's an oncology nurse.
She was an oncology nurse.
She showed me a trick to get a
kid to sit still for an IV once.
So this was just random?
Is this the start of something?
INGRID: Nobody knows.
They found the rooftop
the guy shot from.
Every other vantage point with
a view of the hospital is clear.
This is all less than two hours old.
Police will be at every entrance
until we learn more.
(PHONES CHIMING)
It's Watson.
You should be in Baltimore.
I didn't hear you come in.
Thank you for treating Holmes.
But I did look at his head imaging,
and I don't think he has Wernicke's.
With respect, Watson,
I don't think you should be
worrying about
that version of Sherlock Holmes.
You should be thinking about the one
that lived inside
your head for months.
Respectfully, this isn't a discussion
about my condition.
It's a differential.
ADAM: Who are you to say that?
You should be with Lauren.
Oh, you're gonna tell me
where I'm supposed to be?
I named my son after you.
I gave him your name.
- And I'm honored by that.
- ADAM: Are you?
Because it seems like
you couldn't care less.
It seems like, if you had
an ounce of respect for us,
after all we've been through,
you'd be where you're
supposed to be, in the hospital.
And you'd trust us to take care
of Sherlock Holmes.
Hey. Maybe we should keep
our voices down.
We have a patient who needs to rest.
I respect every single one of you.
I admire you.
If you need me to say that I love you,
I will say that, too.
But Sherlock Holmes is sick.
I'm not leaving.
You can walk out that door,
or you can help.
10% of Wernicke's patients
have a normal CT.
Yes, his blood alcohol was zero,
but his blood work
confirmed elevated liver enzymes.
SASHA: If we're right, the
thiamine we administered will
keep this from developing into
Wernicke-Korsakoff syndrome.
Either way,
Sherlock is getting better.
Yeah, I'm not ruling Wernicke's
out, but if he does have that,
then Sherlock's memory
should be getting better by now.
So, let's order a CK, ammonia level,
a BHB, a lumbar puncture, and an MRI.
HOLMES: Hello there.
You look familiar.
Are you my doctor?
SHINWELL: All right. Here we are, guv.
(BRAKES SQUEAL)
(ENGINE TURNS OFF)
(SIGHS)
I'm sorry. Wh-What were you saying?
Nothing that can't wait, guv.
Actually, guv.
I do have a bit of news. Uh
It feels wrong saying it out loud,
what with all that's happening, but
(SIGHS)
I'm to be married.
- (CHUCKLES)
- What?
It's true.
As unlikely as it may seem,
that's a true fact.
(BOTH LAUGH)
Look at you. Congratulations.
Carlin's lucky. You're both lucky.
- Wh-What're you doing?
- Going up there with you.
Make sure there's no one
lurking around the flat.
Do you think it a coincidence
that Mr. Holmes just shows up
the night before that poor
young woman gets gunned down?
I mean, I suppose it could be,
but you know what the great man
said about coincidences.
(CAR DOOR CLOSES)
The universe isn't that lazy.

It's only temporary.
You don't have to explain
yourself. I'm not mad.
Not anymore, anyway.
I want to help you, John.
Before I tell everyone
I'm leaving UHOP, I was
hoping to have you
I don't know taken care of.
(CHUCKLES)
Dr. Niles is still willing
to work with you.
He'll even come up to Pittsburgh
to do the surgery. Call him.
If that's something you want,
call him.
He looks different
than I pictured him.
- Sherlock Holmes.
- You saw him?
We talked for 20 minutes.
He's charming.
- There you go.
- HOLMES: Thank you.
Good morning, Watson.
I understand I'm quite unwell.
Are you really just gonna
linger in the doorway?
Look, I'm sorry. I just
I-I can't believe it's you.
(BOTH LAUGHING)
(LAUGHING) I can't believe
you're really here.
So where have you been, Holmes?
Oh, I can't remember very much.
The falls, of course.
I know I wanted to fake my own death.
I know I went
into the falls after you.
After that
it's all quite foggy really.
I do remember a room.
A patch of sun moving across the floor
as I worked from dawn to dusk.
Even before the falls,
there's blank spots.
Whatever this is,
whatever's wrong with me,
I wonder
I wonder if it's been there
for quite some time.
Will they come back?
With treatment,
will my memories come back?
It's hard to say.
The electrolytes, the thiamine,
they have you stable for now, but
we don't really know
what's wrong with you yet.
The doctors on my team,
they suspect that it's
it's, um, it's-it's-it's Warner's?
(DISTORTED WHOOSHING)
No, no, it's Verner's?
(ECHOING): Verner's
Verner's Verner's
I I don't remember
the name right now, but
Are you sick, my friend?
No. That was just a brain fart.
And yet, you've recently
been admitted to a hospital
- in Baltimore.
- How'd you know that?
Not my finest deduction,
but I'm not above plucking
low-hanging fruit.
Oh. This
- So, I have a tumor in my brain.
- What?
It's called a glioblastoma.
- It requires surgery.
- Watson
Listen. I'm gonna get
an operation soon, very soon,
but we just have to figure out
what's wrong with you first.
Would you object
if I asked you a question?
No. Of course not.
Watson.
You've already taken me
quite a long way.
I'm in the care of doctors
hand-selected by you, my friend.
My dearest friend.
Is it me you should be looking after
or is it yourself?
NILES: This tumor is
going to give you pain,
memory loss, seizures.
It's growing fast,
and it wants you dead.
(VENDING MACHINE BEEPS)
SEBASTIAN: Yes. Thank you.
(BAG DROPS)
Voilà.
I'm sorry I'm disturbing you.
You seem to be sitting
under a dark cloud.
- Do you work here?
- I would love to have worked here.
My mother would've killed
to have had a doctor for a son.
But school was not good to me.
The words and letters
would not behave themselves.
They'd jumble in my mind.
By the time I'd worked out
how to get around that,
well, certain careers
were not open to me.
What is this? Who are you?
I am
Colonel Sebastian Moran.
Yes, you've heard of me.
You worked for Moriarty. You
did anything he asked you to.
Well, certain careers
were not open to me.
Did you murder Luna Quinn?
I did.
Rather extravagant
as an opening gambit,
but I wanted to get
your attention, and I see that
I have.
You see, Sherlock Holmes
has been with me.
He's not quite the same man
as he used to be,
but still quite useful.
An "idiot savant,"
I think you call it, since
he wandered back to London.
What have you been doing with him?
Solving problems.
There's been quite a power vacuum
since our dear leader,
Professor Moriarty,
shuffled off this mortal coil,
in your establishment.
Your Mr. Holmes, recently,
he's been sicker, more addled.
Not quite so useful.
You brought Sherlock here.
I did. I want him patched up,
and then, I want him back.
So, your problem's gonna have to
take a back seat for the moment.
That's not gonna happen.
My symptoms, they
they've been getting bad.
Gonna have to manage
because the stakes are higher
than you think.
Now, do not do anything rash.
Do not grab your phone
when I leave this room.
We have eyes on Dr. Morstan
at this very moment.
So, your choice is not between
your life and Sherlock's.
It's between Sherlock's life and hers.
- I'm not going anywhere.
- Mary, you don't understand.
Sebastian Moran killed
Luna Quinn yesterday.
- So we bring in the police.
- We can't do that. Not yet.
Moran is not alone on this.
I need to work with Sherlock to
figure out where his people are.
The only way
that you are gonna be safe
is if we get them all at once.
This is still my hospital, John.
One of our nurses
was gunned down yesterday.
I am not hiding. I am not running.
- Mary
- Oh, it's frustrating, isn't it?
Someone you care about,
refusing your advice
about how to stay safe?
(DISTORTED WHOOSHING)
SASHA: Watson? Are you okay?
Yeah, I'm-I'm here.
I'm fine. I'm-I'm here.
Did I write that?
Sorry. Obviously, it was me.
All of these-these symptoms,
they're behaving like CTX.
But that's impossible. CTX is genetic.
If Sherlock had it, he would've
had symptoms his whole life.
I'm aware of that.
It's acting like CTX.
It can't be CTX. So what is it?
I still think it could be Wernicke.
Maybe, but he's showing white-matter
T2/FLAIR hyperintensities.
Look at his MRI.
(SIGHS)
If they are lesions,
they're near the ventricles.
I'd say MS, but there's
no Dawson's finger pattern.
It's too symmetric.
- Atypical MS.
- That tracks. That could be it.
Put him on methylprednisolone.
And if we are right,
if this is in fact atypical MS,
Holmes should improve quickly.
INGRID: You're reviewing
the blood samples?
We have a diagnosis. Atypical MS.
SASHA: A tentative diagnosis.
We've been doing this for a while.
It's always good to have
a backup theory.
You're smiling?
Everyone signed off.
I'm back in my therapy group.
Congratulations. I'm happy for you.
You're not smiling.
(SIGHS) I'm just a little busy.
I don't mean right now.
I mean, in general.
You're still not smiling.
What is this?
That's everything I learned
about your mother.
Your birth mother.
She's real, Sasha.
Her name is actually Shun Yi.
She might not have reached out to you,
but she's right there,
if you want to take a chance.
(CRYING) Thank you.
I appreciate this.
(INDISTINCT P.A. ANNOUNCEMENT)
WATSON: You okay?
Been worried about you, Shinwell.
I know who Sebastian Moran is to you.
I know what he did.
It's academic anyway, innit?
I ain't got the foggiest idea
where Moran is.
Sebastian Moran?
If it's Moran you're looking for,
X marks the spot.
You'll feel better soon, guv.
I'll thank you not to condescend
to me, Shinwell Johnson.
Sebastian Moran has dyslexia.
Yeah, he pretty much told me that.
It led the world
to underestimate the man,
so keen mind that he is,
Moran developed a work-around.
Certain letter pairs are confusing
to someone with that condition.
P and Q. M and W.
Moran's solution was to replace
those letters with an X.
Work out the sounds around them,
and then make the right choice
using context.
How do you know that?
HOLMES: Because the man has a tell.
He gravitates toward the letter
that makes him feel safe.
The letter X shows up
in Moran's speech,
and his writing,
far more than is typical.
Far more than he realizes.
If Moran is operating here,
then he's staying here.
If he's staying here,
he's using an assumed name.
A name
that probably has at least,
what, two Xs in it, right?
Shinwell, call Detective
Lestrade. Tell her to check
- the guest lists at all the hotels.
- (MONITOR BEEPING)
SHINWELL: What's happened?
WATSON: Sherlock?
SHINWELL: His heart just stopped.
(MONITOR FLATLINING)
No pulse. Start the compressions.
(ALARM RINGING)
Code Blue! We have a Code Blue!
- Status?
- One minute down, no change.
CARLIN: Clear.
- (THUMPS)
- (MONITOR BEEPS)
He's still in V-fib. Again.
Charging.
Clear.
- (THUMPS)
- (MONITOR BEEPS)
(GASPS)
We've got him.
Let's keep him stabilized
and get a full cardiac panel.
Where's John?
Anyone see where Dr. Watson went?
Guv?
Where'd you get
Ah, John. Help!
Dr. Watson's down!
Come on, we need help now!
- SHINWELL: We need help now!
- (GROANING, STRAINING)
Keep his head protected.
Shoulders only.
MARY: I need everybody to stay back.
What's the duration
of the seizure so far?
I don't know. Three, four minutes?
I lost sight of him.
We need buccal midazolam.
Top drawer of the crash cart.
Hurry.
I've got you, John.
Can you hear me?
- I've got you.
- Mary.
(SIGHS)
SHINWELL: There's nothing.
- There's no change. No, it's not stopping.
- Give it a second.
Just give it a second.
His seizure's stopped.
Your EEG. It's nightmare fuel.
Dr. Derian.
How long have you been sitting there?
You're having prolonged
cortical discharges.
Your brain is firing
like it's stuck in a loop.
How's Sherlock?
I'm not here to talk
about Sherlock Holmes.
He's not great.
His angio confirms
premature atherosclerosis.
He has narrow vessels,
even though he has an optimal
lipid profile.
He's also fighting for breath.
Atypical MS shouldn't cause
heart attacks.
Are you listening to me?
Are you finally, maybe listening
to me, even just a little bit?
You just had a tonic-clonic seizure.
And it won't be the last one.
Your best case scenario is more
seizures just like that one,
wherever and whenever your brain
finds it convenient.
I know you're aware of the worst case.
- Status epilepticus.
- That's right.
You're in danger of having
a seizure that doesn't stop.
It's often fatal.
And even if it isn't,
a seizure like that
carves out more and more of you
the longer it goes on.
Language. Memory.
Everything that makes you you.
Dr. Watson.
John.
You mean something to me.
You gave me a place in this awful
beautiful world,
and if I have to visit you
at a long-term care facility,
every day, forever,
I'll do it.
But please
Please, don't make me.
CARLIN: You did good.
With the seizure.
Good job.
We got a clue.
Before everything blew up,
Mr. Holmes gave us a clue
about where we can find the man
that shot Luna Quinn.
That's good.
I'm sure the police
are really grateful
to have any help they can get.
That's what Dr. Watson said.
Tell the police.
- Did you?
- This man
The man that killed Nurse Quinn.
He owes me a debt.
I asked you to marry me
because you told me
that the man who went collecting
on debts like that was dead.
You promised me.
So I did.
You can still call Lestrade.
You could do it right now.
(HOLMES BREATHING HEAVILY)
Watson?
You've come back.
They told me you had a seizure.
I did.
Good God, man. The pair of us.
We're crumbling
like an old Saxon fort.
Will it be morning soon?
Yeah, the sun will be coming up
any minute now.
I wouldn't mind
catching a glimpse of that.
Don't know if I have another
sunrise in me, my friend.
ADAM: Lauren's hand cream.
Vegan what-have-you snacks.
Yes. My AirPods.
You are the man.
Stephens?
Sorry.
I got caught up just staring.
They are amazing, aren't they?
Stephens.
Did our father kill himself?
I didn't even know
you wondered about that.
Yeah, I think he did.
I think he did, too.
I want you to promise me something.
If that's ever you,
tell me you'll stop.
Tell me you'll call me.
You're my brother.
You're my twin.
I can't lose you.
Can I tell you something?
I'm not worried about that.
Not anymore.
I can see
I don't know, call it a future?
I'm gonna be alive.
I'm gonna have a life.
You already do.
HOLMES: I was promised
sunlight. This is just
- cold and murk.
- (SCOFFS) Oh, please.
- We've sat through much worse.
- (CHUCKLES)
I like this place.
What I can see of it.
WATSON: It was right down there,
my mother used to work at that museum.
There's a library right next to it.
Well, actually, they're attached,
if you can find the right hallways.
I'd walk there after school,
wait till she finished work.
Must've read half the books
in that place.
Time well invested.
What a lucky thing for your city.
Lucky thing for all of us.
What are you thinking about?
The Cobalt Fissure. It's odd.
We both chased Alexander Petrov
across that blasted heath.
We both absorbed whatever radiation
was lingering from that
weapons site, and yet,
you have a brain tumor.
And I got away scot-free.
This your idea of scot-free?
This is something else.
We both got sick,
but we didn't get sick together.
but we didn't get sick together.
Ah. Look at that.
Creation itself.
A passion play,
staged anew every morning.
I'm sorry.
I was thinking about something.
Holmes, that cloth that you're using
to wipe the sweat from your fever.
Can I borrow it?
(FIZZING)
(LAUGHING)
(SIGHS)

(DISTORTED WHOOSHING)

(INDISTINCT P.A. ANNOUNCEMENT)
SASHA: It happened.
The seizure that never stops.
Watson's in a coma?
Not exactly.
The medications,
they mimic that kind of state.
We'll wean him gradually,
see if the seizure activity
has stopped.
And if it doesn't?
Even if the seizures do stop,
who's to say what's left in there?
(DOOR OPENS)
(DOOR CLOSES)
Alexander Knox.
'Twas the Xs that done give you away.
SEBASTIAN: Ah.
- Well done there.
- Yeah, don't bother.
I've swept for weapons.
The only thing you're gonna find
stashed there
is overpriced bourbon.
Fair enough.
And why is it that you are here
and not the police?
You know why.
Luna Quinn.
Nancy and William Evans.
Countless others.
Ms. Quinn was unfortunate,
but necessary.
As for the old couple, what
makes you think they're dead?
Ah, I don't know.
Maybe the blood splattered
on the walls of their flat,
the scene of the slaughter.
Stagecraft.
Why would I give up
such a piece of leverage
with someone like yourself?
The Evanses are fine and dandy,
but you have done well
to track me down to here.
I cannot let you go empty-handed.
I will give you my Lieutenants
Maitland and Ort.
One of them is very close
to Dr. Morstan as we speak.
Those men trust you.
But we are all of us alone
in the end, Shinwell.
You know that.
And congratulations.
You're engaged.
The odd couple,
but quite suited in the end.
So what is it?
I'm to be beaten to a bloody pulp
for a couple of murders
that may not have happened,
or you hand me over to the police,
and then, they definitely will happen.
Or you let me leave.
Go back to the life you were living.
Untainted by violence.
HOLMES: Squeeze my
hand if you can hear me.
Squeeze my hand if you're Watson.
(CHUCKLES) Excellent.
You may notice I'm looking better.
Well, your team found
the results of your experiment.
The one you were working on
when you collapsed.
You trained them well, my friend,
because they filled in
the blanks perfectly.
CTX.
(CHUCKLES) You needn't
crush my hand, John.
I'm aware it's a CTX phenocopy mimic.
Seems I'm a mutant.
My genes altered by the
radiation of the Cobalt Fissure.
All my symptoms the, uh,
confusion, the heart attack,
the blindness
they came from
a cholesterol-like buildup.
So, they're administering
something called, um
Well, there's not many words
I won't attempt,
but your young doctors
managed to find one.
And I must say, it's worked a treat.
We did get sick
at the same time, my friend.
The radiation that caused
your tumor altered my genes.
Alike, even in illness,
Holmes and Watson.
(ELEVATOR BELL DINGS)
Two conspirators arrested.
Moran is in custody.
What is that?
(SIGHS)
I knocked out four of Moran's teeth.
When I got to his molars,
he told me what I needed to hear,
the location
of William and Nancy Evans.
And yeah.
Yeah, I wanted to carry on.
Good God, I wanted to carry on.
But I didn't.
I called Detective Lestrade and
told her where to find Moran.
No one was damaged beyond repair.
It's just
I
I can't lose this.
I can't.
If there's still a home here
for me then
then I had to come home.
SASHA: Hey.
Usually, you find me at my desk
when you want to talk.
I know. I like it here.
I want to try.
I'm so sorry I didn't hear you
when you tried to bring up kids.
But I hear you now.
And I want it, too.
I want that and
whatever that means for you.
If that means a big day,
300 people in ball gowns and tuxes,
that ceremony that
you've had in your head
ever since you were nine years old,
I want to give that to you.
Hmm.
I'm not sure how much I trust
the things in my head right now.
Are you talking about Beck?
Because that was just a blip.
In the course of your life,
it's just a thing that happened.
It doesn't have to change anything.
I'm an optimistic person.
I put out into the world
exactly what I want it to be.
- And I love that about you.
- Yeah, but I've
I've been a bit of a fool.
For a good while now.
I need to change some things.
I don't know how
I'm supposed to be in the world.
I don't know who I am.
And if I don't know who I am,
how am I supposed to know
who to be with?
Stephens, I'm happy you know
what you want right now.
It just shouldn't be with me.
(CRYING)
Good morning, Dr. Derian.
I already answered your questions.
I'm not here to ask any questions.
I'm here to show you something.
Did you know Beck Wythe kept
a hidden camera in his condo?
Took us a minute to find it,
but it turns out,
it was running
the night you stopped by.
I don't want to watch this.
He must've known you were
interested in that laptop.
So, the events in question.
They're all right there.
That's it.
That's exactly how
I told you it happened.
